COVID-19 Waiver, Screening Form, & Consent

By signing and submitting this waiver, I am confirming the following self-assessment measures have been met.

ACKNOWLEDGEMENT

These procedures are subject to change based on the most up to date guidance from Health Canada and the Ontario Ministry of Health.

I, the undersigned acknowledge and agree that I have:

1. Read and understood this memorandum.
2. I am aware that Tara McKay High Performance Skating Inc. and its staff cannot guarantee that I will not contract COVID-19 while participating in the programs. I am participating at my own risk. In particular, I accept the risk of contracting COVID-19 should I attend Tara McKay High Performance Skating Inc. programs
3. Freely and voluntarily accepted the risk of participation.

Parents or court-appointed legal guardians must be 18 years of age or older and must authorize on behalf of any participating minors and agree that they and the minors are subject to all the terms of this waiver as set forth herein.

The undersigned agrees to abide by these procedures and to disclose any facts indicating any higher than usual risk of having been exposed to or contracted COVID-19 and to indemnify and save harmless Tara McKay and Tara McKay High Performance Skating Inc from any and all liability in connection with my participation.

Please note, if you answer YES to any of these questions, you will not be granted permission to enter. Please call your local heath unit or health care provider.
